Excessive gingival display can be one of the most distracting characteristics of many of our patients’ smiles, but traditionally we have had very few options to solve this problem outside of invasive orthognathic surgery. Many dentists are not willing to recommend this invasive surgery to their patients for a cosmetic procedure, and thus the patient is left to deal with this unaesthetic condition. Control of gingival display is not limited to removal of gingival tissue. There are multiple causes that include tooth position, vertical dimension, musculature, as well as clinical crown length considerations.
A weakness of orthodontics in the past has been vertical control of the occlusal plane that often results in gingival display problems even in cases that did not start with this unaesthetic result. Now, with new tools available, we can control the vertical, and thus in conjunction with other tools predictably control the amount of gingiva showing in the smile. Techniques including gingivectomy and/or the use of bone screw anchorage now enable us to reduce extreme cases of gingival display to the point that orthognathic surgery to impact maxillas and reduce VME has become obsolete.
