Corticotomies

Corticotomies are a great surgical alternative for dealing with skeletal resistance. The corticotomy has been proven to be a very valuable tool in its years of being performed, especially since it is an easy procedure to perform and minimally invasive to the patient. However, many are afraid to perform this simple surgery, and many surgeons are not knowledgeable on the procedure and thus will not perform it.

Dr. David Dana has put together a hands-on course to eliminate this fear, and give confidence to you and your team on how to perform this simple surgery. Dr. Dana will go through step-by-step on how the incisions are to be made, the relevant structures to be concerned about, and the literature behind the procedure. Students will have the opportunity to work on pig mandibles to further practice the technique under the guidance of Dr. Dana. There is nothing to fear from the corticotomy...it is a simple surgery that any POS student can perform. Come learn the details of this procedure, and open up your practice to giving your patients the best treatment alternative for every case.

Topics:

• Understanding facial balance and esthetics

• Skeletal malocclusion and how to deal with them without surgery

• Orthodontic Principles

• Historical Review and literature (including William H. Bell)

• Anatomy to be concerned about

• Flap Design for each type

• Hands-on suturing techniques on pig jaws

Q&As with Dr. Dana:

Is it invasive?
No. It can be compared to a quadrant of periodontal osseous surgery. It is done in the office with local anesthesia.
What will it do?
It will allow you to correct significant malocclusions, class II and class III with orthodontic treatment alone, will allow you to make dental corrections of over 10 mm, with excellent control of the torque and vertical dimesion. Will enable you to correct posterior cross bites on adult patient without the need for midpalatal surture fracture. 8 out of 10 patients will refuse orthognatic surgery to correct their skeletal problems. 10 of 10 patients in my practice accept the alternative of corticotomy.

What is a Corticotomy exactly?

By making small cuts on the cortical plate (1-2 mm) around teeth that require to be moved beyond what we consider normal limits, we are able to distract the cortical plate and move the teeth in a safe manner far beyond what was ever thought possible. Many orthodontists today use this procedure to speed up their treatments. Others like me also use it to treat patients with significant skeletal malocclusions, that if not for the corticotomy would need to be treated with orthognatic surgery.
